Job Description:
A company is looking for a Patient Engagement Specialist to
connect patients with essential health and social services.
Key Responsibilities
Conduct high-volume outreach to educate patients and caregivers on
health services and assist in scheduling appointments
Organize and prioritize workload to meet outreach and enrollment
goals while documenting interactions accurately
Collaborate with care teams to ensure smooth transitions from
outreach to ongoing support and provide feedback on strategies
Required Qualifications
2+ years of experience in customer service/sales, preferably with
cold outreach experience
2+ years of experience in healthcare outreach, community
engagement, or related fields
Experience in healthcare, social services, or mental health
services, with an understanding of seniors' needs
Basic proficiency in G-Suite and video conferencing platforms
Strong tech proficiency and comfort using technology in daily
work
