A company is looking for a Care Coordinator - Care
Management.
Key Responsibilities
Coordinate care and provide guidance, support, and education to
patients
Collaborate with the care team to develop and implement
comprehensive care plans
Advocate for patients to ensure they receive necessary care and
services
Required Qualifications, Training, and Education
Bachelor's degree
Minimum of one year in current position for internal applicants
Healthcare and/or community-based experience with diverse
populations preferred
Bilingual (English/Spanish) preferred
Proficiency in MS Office and ability to learn care coordination
applications
