part of, we would love to have you. The Staff Pharmacist works to
support the mission and vision of Neighborhood Healthcare by
providing excellent patient-centered care and services within a
dynamic pharmacy team environment. This role will utilize
pharmacist skills such as assessment, critical thinking, judgement,
decision making in reviewing and dispensing prescriptions. This is
a true per diem position with variable, non-guaranteed hours based
on PTO coverage needs of one full-time pharmacist. This position
does not offer a minimum number of hours per week and is not
eligible for benefits. Schedule will be Monday-Friday between the
hours of 8am-5pm. Responsibilities Reviews prescriptions issued by
the Physician, or other authorized prescriber to ensure accuracy
and appropriateness Examines medication history to determine if
there are contraindications or significant interactions, and make
appropriate recommendations for change Ensures accurate
verification and dispensing, including proper dosages and
indications Provides patient consultation for all new
prescriptions, when requested by the patient, or when in the
patient’s best interest Provides oversight of Pharmacy Technicians,
Interns and ancillary personnel Directs pharmacy staff to engaged
in typing, filling, and labeling pharmaceuticals on matters
pertaining to the pharmacy and pharmacy law Maintains current
knowledge of all applicable federal and California state pharmacy
laws Substitutes therapeutically equivalent medications in
accordance with health plan formularies, pharmacy formulary and
interchange protocol, independently of the provider Documents all
therapeutic interchanges in the medical record and pharmacy record
Refills medications on behalf of providers according to
Neighborhood’s Pharmacist-Nurse Refill Protocol Orders labs needed
to monitor medications with the Neighborhood Pharmacist-Nurse
Refill Protocol Answers clinical questions from providers
accurately, promptly, and thoroughly using the most up to date
clinical guidelines and evidence Communicates via the EMR with
other Neighborhood staff as it relates to patient care Attends and
participates in required meetings and trainings Performs all job
functions with Neighborhood’s mission, vision, values, and
strategic plan in mind Quality Management Contributes to the
success of the organization by participating in quality improvement
activities Cooperates in accomplishing department goals and
objectives Customer Relations Responds promptly and with caring
actions to patients and employees Maintains professional working
relationship with all levels of staff, patients, and the public
Qualifications Education/Experience Minimum 1 year of experience in
a retail pharmacy setting required Pharmacy doctorate degree from
an ACPE accredited pharmacy school required Valid CA pharmacist
license required Current CA DEA licensure is required and is to be
maintained as a condition of employment Current Basic Life Support
(BLS) certification through an American Heart Association (AHA)
approved source is required upon hire and must be maintained as a
condition of employment. AHA-approved courses include an in-person,
hands-on skills check with a certified instructor using a mannequin
to demonstrate CPR and emergency response techniques. Online-only
BLS courses without a live skills check do not meet this
requirement. Pharmacy practice residency preferred Board
Certification Pharmacotherapy Specialist or other specialty
preferred Bilingual (English/Spanish) preferred Additional
Qualifications (Knowledge, Skills, and Abilities) Excellent verbal
and written communication skills, including superior composition,
typing, and proofreading skills Ability to interpret a variety of
instructions in written, oral, diagram, or schedule form
Knowledgeable about and experience with pharmacy practices and
protocols Ability to successfully manage multiple tasks
simultaneously Excellent planning and organizational ability
Ability to work as part of a team as well as independently Ability
to work with highly confidential information in a professional and
ethical manner Physical Requirements Ability to lift/carry 20
lbs/weight Ability to stand for long periods of time Compliance
(Safety and HIPPA) Maintain current knowledge of policies and
procedures as they relate to safe work practices Follow the NHC
Pharmacist-Nurse Refill Protocol for refill authorizations Follows
all safety procedures as outlined in Neighborhood Healthcare’s
Illness and Injury Prevention Plan (IIPP) and report any injuries
and/or unsafe conditions immediately Maintains current knowledge of
policies and procedures as they relate to safe work practices
Follows all safety procedures and report unsafe conditions Uses
appropriate body mechanics to ensure an injury free environment
Familiarity with location of nearest fire extinguisher and
emergency exits Follows all infection control procedures including
blood-borne pathogen protocols Maintains privacy of all patients,
employee and volunteer information and access such information only
on a need-to-know basis for business purposes Complies with all
regulations regarding corporate integrity and security obligations
Reports all behavior and/or activity that are unethical,
fraudulent, or unlawful Pay range: $65.09 to $101.53 per hour,
depending on experience. Compensation Disclosure: The posted salary
range reflects the designated pay grade for this position. While
this range represents the broader classification of the role,
actual compensation will be based on several factors, including but
not limited to: the candidate’s overall knowledge, skills, and
experience, market data and industry benchmarks, internal equity
within the organization, Budgetary considerations and
organizational needs. As a result, placement within the range is
not guaranteed, and the full pay grade range may not be
utilized.
